Responsible for training and conducting outreach program activities and health education for participants and their families in identified counties throughout Tennessee and surrounding areas to help maximize and maintain healthy lifestyles. Maintains and documents accurate data related to the outreach programs. Models appropriate behavior as exemplified in MLH Mission, Vision and Values. Presents health information to patients, families, or community groups in order to promote healthy lifestyle decisions and chronic disease management. Provides health education and awareness to the assigned community program. Enters data, documents and evaluates the effectiveness of program activities. Requires a High School Diploma or Equivalent, or Bachelor's Degree in Health Administration, Education, or Social Services. Requires 1-3 years of customer service, collections or call center experience. Proficiency in Microsoft Office, ability to operate standard office equipment, compose complex written materials, exercise independent judgment, assess needs of participants and families, strong organizational and communication skills. Physical demands include light work with occasional exertion up to 25 lbs, frequent travel, exposure to patient bodily fluids and hazardous materials. No supervisory responsibilities assigned.
