The Crox Group is seeking a local contract nurse RN Case Management for a local contract nursing job in Santa Rosa, California.

SKILLS:

• 2-year acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment - required
• clinician must have current work history within 2 years.
• Computerized documentation, required.
• Ability to organize and participate in family meetings
• Participate in the development of the treatment plan.
• Experience with HMOs, IPAs, etc.
• Competent with arranging DME, SNF and ambulance transport. Must be able to perform this task independently.
• Ambulatory, Behavioral Health, Home Health, Hospice, Insurance Company, Long Term Acute Care/Rehab/Skilled Nursing experience PREFERRED
• Acute Hospital Experience by Specialty like ER, ICU, NICU, Pedia, SDU/PCU/IMC PREFERRED - Experience with MS is required

Case Management/Utilization Review Experience:

• Prior Authorizations - required
• Plan of Care - required
• Needs Assessment/ Order DME - required
• Discharge Planning - required
• Care coordination - required
• Benefits Eligibility - required
• Admission Criteria - required
• Needs Assessment/ Order DME - preferred
• Plan of Care - preferred
• Prior Authorizations - preferred

Regulatory Experience:

• CMS: Centers for Medicare and Medicaid Services - required
• DRG (Diagnosis Related Groups). - required
• HIPAA (The Health Insurance Portability and Accountability Act) - required
• OSHA: Occupational Safety and Health Administration - required
• The Joint Commission/ Core Measure/National Safety Goals - required
• CPT (Current Procedural Terminology) coding and billing - preferred
• Department of Health - preferred
• ICD 10 Coding - preferred
• NCQA (National Committee for Quality Assurance) - preferred
• Worker’s Compensation - preferred

Certifications: REQUIRED

• BLS
